What the contact form collects
When you use the contact form, I receive what you type in:
- Your name and email address
- Your practice or company name, if you give it
- Your city, state or province, and country, if you give them
- Your message
Please don't include patient information or other health records in the form.
How it's used
I use what you send only to reply to you and to talk about the work you asked about. I don't sell it, rent it, or add you to a mailing list.
Hosting
The site and the contact form run on Cloudflare. Like any web host, Cloudflare processes basic technical information, such as your IP address and browser type, to deliver the site and protect it from abuse. There are no ads or ad trackers on this site.
